CPT® is a registered trademark of the American Medical Association.

The following copyright notice is included on each page, or as often as reasonably practical, of any display or print-out where CPT® content appears (other than that which would constitute fair use, internal reports, and claim forms for specific patients).

The trademark symbol ® follows the first appearance of "CPT" in each section of the Licensed Products.

(d)

When the Licensed Products are updated with subsequent annual releases of Licensed Content, the copyright year is updated as specified in the Licensed Content, which is usually the year prior to the title date (e.g., the copyright year for CPT® 2026 is 2025).

II. Other Notices and Disclaimers

(a) U.S. Government End Users

CPT® is commercial technical data, which was developed exclusively at private expense by the American Medical Association (AMA), 330 North Wabash Avenue, Chicago, Illinois 60611. Use of CPT® in connection with this product shall not be construed to grant the Federal Government a direct license to use CPT® based on FAR 52.227-14 (Data Rights - General) and DFARS 252.227-7015 (Technical Data - Commercial Items).

(b) National Correct Coding Policy

The responsibility for the content of any "National Correct Coding Policy" included in this product is with the Centers for Medicare and Medicaid Services and no endorsement by the AMA is intended or should be implied. The AMA disclaims responsibility for any consequences or liability attributable to or related to any use, nonuse or interpretation of information contained in this product.
